Application Procedure

Once you have selected your desired courses or workshops, download the application form and complete the following registration procedure:

• Review the certification policies carefully and sign the Certification Policies Declaration.
  
• Complete and sign the application form, clearly indicating the desired certification course(s).
  
• Submit your application and deposit, in person or by e-mail, fax, or regular mail to:

All applications will be processed in a timely manner, and students will be notified of their acceptance by phone or e-mail. 
Please note: A $300 deposit per course is due at the time of application. The deposit fee will be charged as soon as the application 
has been reviewed.  The deposit is non-refundable and non-transferrable.
